Legal Question in Wills and Trusts in Illinois

Do I need an attorney in order to file paperwork for Standby/Successor Guardianship in Illinois? This would be supported by family members (the parents were made guardians of the disabled adult last year) and the paperwork is a simple form (https://www.illinois.gov/sites/gac/Forms/Documents/sbgf.pdf). Thank you!
