Legal Question in Wills and Trusts in Illinois

Is there a lawyer from Illinois who would be willing to look over these 2 PDF documents? They are regarding:

A Gift for my Loved Ones-

"This package contains everything you need to know if I am disabled, or incapacitated and cannot make decisions for myself. It includes my Health Care Power of Attorney and Advance Directive for Health Care - Living Will"

I just want to know if they would be considered legal and valid in the state of Illinois AND if I needed a lawyer to sign them in order for them to be valid or is a notary enough? I downloaded them off the website of the Orthodox Church of America which I believe is based out of New York.

It is unlikely you will get anyone to click on a link to an unknown website. Even if someone was so inclined, I would guess the cost of reviewing an unfamiliar document would be as much or more than having an attorney prepare the documents you need. Kind of like a sign I saw in a bicycle shop years ago: "Labor $25 per hour. $50 per hour if you worked on it first." It is often more time-consuming to review someone else's work than it is to use a document which you already have reviewed. You may want to contact some estate planning attorneys in your area and either take the documents to them to review or have them prepare the documents you need.
