Legal Question in Credit and Debt Law in Indiana

I just received a statement from (liberty medical supply, ins) amount stated i owe is as stated on statement is for 30.75 and said date is from 2/27/2009.

i have never , ever , received a statement for any amount from these people in the past. i did at one time, use liberty medical supply to get my diabetic testing supplies. insurance always pay'd.

i called the number on statement (1 800 840 3844) they told me they had trouble getting statements mailed out for a while. (not my fault). i dont know if to beleive that or not.

my question is, am i responsible after all that time to pay that bill?
