Changing name on birth certificate

My grandfather has asked that my mother change her last name on her birth certificate. She was given her biological fathers name of 'Jones' when she was born. My grandmother got married when my mom around 2 years old. The family name became Phillips. My grandfather is climbing in age and would like to see that my mom's name be Phillips on her birth certificate. My mom is happily married and does not want to change her existing married name of--name removed-- she only wishes to change it on her birth records. This request is to comfort my grandfather. What are the steps that need to be taken?

Re: Changing name on birth certificate

You will need to get a court order for a name change and then forward that to the Indiana Department of Health,Vital Records Department Corrections Division in Indianapolis. There number is 317.233.2700. There is a fee of $18.00. Call them for specifics.
