One in the same affidavit form

I'm trying to find a form that states that a persons name that is on some judgments is the same name as mine but i am not that person so that i can have it notorized what type of form is this and where can i find it on the internet or somewhere else.

The only form that I am aware of is a personal information form used by title companies when a buyer or seller has the same name as someone who has a judgment.
