You need to obtain a from from the Clerk of the Court, complete the form, and have the Clerk sign the completed form. You should read Trial Rule 34 C and also determine how you want to serve the Subpoena as well as what the requirements are concerning originals and copies. You should read Trial Rules 26 - 37.
