Is having my wife's name put on the deed to my home ensure that when I die she will get the home?

If the deed gives you and your wife ownership as tenants by the entireties, husband and wife, then yes, if you die first she will own the whole thing.

Yes, if done properly ensuring that there is a right of survivorship.
