Do I have to register with a state if I wish to start a business as a sports agent. Are there any special forms to fill out?

No, but you sure should, so you can have limited liability, for example as either as an LLC or regular corporation. Also, this is required if your are operating under an assumed name (eg Estherville Enterprises). Also, most cities and towns require business licenses.
