Legal Question in Medical Leave in Iowa
My friend is gay and her partner of 21 years has MS and she is her primary caregiver, but was denied FMLA ,why?

The simplest answer is that they're not family. Federal law does not recognize same sex marriages either, so FMLA would not apply even if they were married in Iowa.
