Legal Question in Employment Law in Iowa

I recently requested one week of vacation leave to assist my mother following her surgery. I have the time coming to me and requested the vacation in the correct format and time required by the company. My employer is denying my vacation unless I supply them with a written request from my mother's surgeon for me to take the time off. They did not mention anything about it being a FMLA situation or ask for any of the paperwork. For me to qualify using FMLA I am required to use all vacation and sick time prior to unpaid leave. If I have vacation time coming am I required to supply my employer with a doctor's excuse if the time requested is not an unpaid FMLA situation? I did not apply for FMLA, just my normal vacation week I have coming. Can my employer demand a doctor's note just for me to take my regular one week vacation just because they are aware what I am doing during that week? Should I fill out the FMLA paperwork?
