In Iowa the first three days an injured worker is off of work (with a doctors excuse) are unpaid, unless the time off of work extends more than 10 days. Once the time off of work extends beyond ten days the worker is paid for the first three days. From a practical standpoint, if its obvious the injury will keep the worker off for 10 or more days the insurance company will not make the injured worker wait but will pay weekly indemnity from day one.
