Sometimes a nursing home will accept a lein on real property fur residents that cannot pay the entire cost if their stay. Byt, much more often it us Medicare that seeks reimbursement by attaching the house. A Tod deed passes what the decedent owned. If their interest was subject to the Medicare lein, that is what would pass to the beneficiary of the deed. Your mother should consult directly with an elder care IR estate planning attorney in her area. It is better that she didn't this well before she moved to a nursing home.

Good luck
