How can I change my child's name if I have no idea where the father is and he hasn't been a part of the child's life in over 4 years?

You should contact an attorney. You should be able to change your child's name given the father's lack of contact, but you will have to have the father served with the name change request. There are ways to do this if you do not know where the father is located.
