The administrative office of the courts (AOC) has a form you can use. AOC 796. It's a medical power of attorney. Go to courts.ky.gov and legal forms. You don't have to use this form, however, it's probably easiest for you. I drafted one just for medical, with an end date, that I use whenever the grandparents take the kids on a trip, or we go on a trip without the kids. Notarized is best, simply because it will look more convincing to a medical provider, although I don't know of any requirement for it to be notarized.
