what kind of an agreement form do i need to write up stating that both parents agree to change custody of a child that is 15 years old

You need to have it in a court order form specifically stating the custody change and any visitation arrangements and signed by both parents. You should submit this to the court as an agreement with a signature line for the judge to sign.
