What is the proper format to reguest that a default judgement be overturned?

In Kentucky, precedence favors parties against whom a default judgment has been entered, and by filing a motion to set aside a default judgment, and citing the proper precedence, a successful argument could be made.

I would strongly recommend, however, that you do not attempt to proceed pro se (without representation), but seek advice from a competent attorney of your choice to proceed with such a motion.
