Legal Question in Personal Injury in Louisiana

I slipped and fell in a dept store.
The accident was caused due to a wet floor(water puddle) that was left on it and not being properly mopped and dried by the maintenance man.
An employee on duty saw the whole incident and came to my aid. She informed the store mgr and the mgr came also. I was told by another employee that an incident report will be filled out. I said ok, but the mgr never even mentioned it. She only asked if I was ok and I stated I think so. I proceeded to checkout then I went home. Well I called the store when the shift changed and informed the am shift mgr of what happened. It seems it was never reported and they then let me file an incident report. Later that day, i started feeling pain in my upper back, neck and shoulder. What should I do now? I dont have any medical insurance and I can'tafford to pay a doctor. I start a new job in the next 2 days.
