How can Trustees be added to a Family Trust?

Can you please tell me the best way to add trustees to the Family Trust?

The Trust document itself should detail who successor trustees can be, and how to have them appointed, but the current trustees would probably need to resign. The beneficiaries may need to approve, and the Probate Court may also need to approve, depending upon the circumstances.
