Birth Certificate Name Change

I recently had to retrieve a copy of my birth certificate from vital records in order to obtain a passport. I discovered that the name on the birth certificate is not my biological father's name. Through asking questions from family members, my mom discovered that when she gave birth to me, her legal guardian at the time gave false information, which was recorded on the birth record. I am no 36 years old. Can you pls. advise how I would go about having this form corrected. I would like for my biological father's name to be placed on my birth certificate. Thank you.

You should speak to the people from vital records about what is required to accomplish this. You will probably need some proof that the biological father is actually your father, although at this late date, I'm not sure how you would obtain this, unless he later admitted paternity in a court proceeding.
