Incapaciated adult guradianship

My sister is my adult brother's guardian in IL and we want to transfer his guradianship to another sister who lives in MD. How do you go about doing this?

You must file a petition to appoint guardian in the circuit court of the county where the petitioner lives. After a notice period, a hearing will be held and if no one objects and the petitioner seems to be fit for the task, the court will issue an order appointing them. It will facilitate matters to get consents in writing from all concerned, especially the IL present guardian.
