Your question was unfortunately devoid of sufficient details to give a complete answer, but if the forgery was on a check, resulting in loss of funds, that is a matter to take to the police. Or you could swear out a criminal complaint before a commissioner at a police station or district courthouse in your county. But you ought to have pretty solid evidence since you will be making the complaint under oath. If this is more of a civil matter, like someone signing your name to a lease or a contract, or some other such document, you should notify the other party involved that you are not the signee.
