removal deceased spouse from deed

In the state of Maryland, what form do I need to fill out

to remove a deceased spouse from a deed? I understand that I need a copy of the death certificate, when I have the new deed recorded.

At such time as you intend to transfer the property to someone else, you sign the deed as "surviving joint tenant" and record the death certificate with the new deed.
