child's name change Massachusetts

I have the opposite situation of most questioners on this subject. My two year old daughter has my last name. her father was not present at her birth or most of her first year. He is listed on her bc and signed paternity papers, but there is no established custody and we are not married. He has since come back into her life and has proven himself to be a loving and responsible father. I would like to have his last name added, keeping mine as well. We both agree...do we still have to petition a court, pay a lawyer, etc? Or can we just file something. What's the cost of something like this?

A change of name must be approved by the court.You file a change of name petition with the probate and family court. A certified birth certificate must be file with the petition along with a filing fee of $150.00.

To change the child's name you need to file a Petition with the court. It should not be too expensive if both you and the father agree to the new name.
