FMLA - Fitness for duty form

in an effort to make things more streamlined and easier for my employees, I am developing a Fitness-for-duty form that the doctor can fill out. Do you have an example that I can use to do this and ensure all applicable information is included? any assistance would be greatly appreciated.

This is a HIPPA question as well as an employment question.

Try asking a state agency for a copy of the form they use, but your best bet is to have an employment attorney with experience in employee benefits grab one from his file...or better yet, find the book the attorneys use for drafting standard documents like that and use that! Most state bars publish guides for attorneys which should be in your local law school library.

Of course, you can also test this against a local doctor, who has filled out a bunch of these, but you can also ask an attorney to help you draft it, so that you get enough information, but not too much information.
