Do the doctors have the right to charge fees for filling out FMLA forms.

Yes, but with at least one possible limitation. You are asking them to perform some service for you so they are entitled to be paid for doing that work. However, if there is something in their agreement with your insurance provider which limits their right to charge for this service, then they would be limited.
