Legal Question in Personal Injury in Massachusetts

if in an accident you are the only one with injuries no citation issued at the time transported to ER no arrest but BOSA is 1.1 and ETOH invoked PD report states driving a little to close and warning given via mail what tests info from ER must be submitted to Insurance company

All information should be provided to insurance company unless you were inclined to want to commit insurance fraud. It is not clear whether you are talking about a third party insurer or your first party insurer, but there is no difference except you would be violating provisions of your policy if you fail to cooperate with your own insurer (that is a valid ground to disclaim).
