can a mortgage originator add PMI 3 weeks after final closing. They say a clerical error.

The PMI was not mentioned in the Good Faith Estimate

It is a violation of TILA and if it is a refinancing and more than $35.00 in the difference it will hurt their ability to foreclose in the future, but that is not your real question.

Is the PMI payment included in the note or mortgage document as part of the payment?

If not, it is their problem. They signed the agreements, they can't modify it now without your consent. I suggest you contact an attorney or call the Attorney General's office for assistance.
