Probate Trust Filings

My disabled client's mother left instructions for his care in a trust. I've found her will in Probate Court, but no documents regarding the trust. Doesn't a trustee have to file in the court a yearly update of where monies are being spent?

One assumes that the trust was created by the words or terms of the will. Is that true?

Was the will probated, and an accounting filed?

What I am trying to reach is what kind of trust, how was it created, and who the trustee is responsible to?
