Turning over a primary residence to a niece

my great uncle promised my mother that he would give her the house if she came to live with him and take care of his then ailing wife. The wife has since passed away. she passed away 2 years ago. Now he is beginning to fail in his health. the house was never signed over to his neice for taking care of his wife. He will have to enter a nursing home in a short period.If he enters into a nursing home will the state of ma. be able to seize the home when his insurance runs out?
