Asking for reasonable accomodation

I understand that there is no form to fill out when asking for an a reasonable accomodation from my employer. Then how do I go about asking for this and making it legal and proper?? I am requesting this in regards to my excerised-induced asthma when I am required to remove snow from my job site. I am asking to be given more time to clear the area which includes 24 emergency exits, 13 entrances and handicap ramp and accompanying sideswalks. My job title is maintenance /custodial and I am able to do all the basic functions of my job.

If your employer does not have a form, you should type up a formal letter to your supervisor and his/her supervisor to ask for a reasonable accomodation. Personally deliver the letter and/or certify mail it.

Describe your job duties, how long they expect you to take to perform the task now and what the task entails. Then, describe your reasonable accomodation and detail why it would not cause the company any undue discomfort, money or time. Describe your disability, and prescriptions you need to take while on the job and the name and address of your doctor. This will let the company know that you're serious and that you are trying to be a good employee. Of course, they will realize that such a well-thought-out letter is also evidence if you have to force them into it later. If any other counsel has something to add, please do, as this list is not supposed to cover everything, just give an idea of where you should head!
