what form do I need to get for so that I am not financially responsible for my mom as she has alzheimers.

In general, you are not responsible for the cost of your mother's care. The key is not to sign anything that will make you responsible. You should talk to an attorney in your state regarding paying for your mother's care. John Peck is a very good elder law attorney in your state.
