what kind of form would my son and his wife need to use to give custody of their son to me if something should happen to them?

Estate planning documents appointing you as the child's guardian should something happen. And they should also appoint an alternative guardian, should you be unavailable. These documents are very important so that family members are not fighting over a child during otherwise difficult times. And if they are fighting, there were at least the parents' intentions in writing, which most courts would honor unless there were extreme circumstances to dictate otherwise.
