What happens if I don't apply for disability benefits

I am on personal medical disability from work. I have work insurance that pays my salary for l year with extended benefits up to 5 years. I have been off for 5 months and told to apply for social security disability. If I am not approved by social security, doesn't my work insurance have to continue my salary amount anyway? I am off for a lung injury that is permanent.

Presuming your employer told you to apply for SSD, the sooner you establish your benefits, the better for you and the employer also. The employer benefits because their disab ins will be reduced by the amount of SSD benefits, so you will receive the same money. If you wait until your disab ins runs out, Soc Sec law could change and it's not changing for the better right now. If you are not approved for SSD, most employer provided disab ins policies are required to pay according to the contractual benefits. Without looking at the policy, I can't tell you what the limitations might be.
