Filing a QDRO

Can an individual file a QDRO, approved by both parties, with the court? Or, do you need a lawyer to do it?

You don't need a lawyer. In most cases, the court would be satisfied upon seeing both parties' approval, provided that the QDRO is otherwise proper and consistent with the divorce decree.
