modifying child support

divorce settled in 2002. Husband filed motion with atty to modify child support. Didn't include financial stmt.INfo. he did add in motion concerning his income is incorrect. I want a form to send him forcing him to produce correct income. Where can I find this form. Along with a form for me to complete for me to attach my affidavit for info I want to submit to be heard at the court hearing. Where can I obtain these forms without paying for them. Also I need to know how many days prior to the hearing do I have to submit my response to the court!!! Thanks

There is no such form. You may request the tax information or serve formalized discovery. However, I know of no forms for that purpose.
