power of attorney or guardianship

I would like to know what form to use. I am living with a widowed father of 3 children, ages 14,11 and 9. We want to make it possible for me to make decisions, medical, school-related, etc. without the father giving up any rights. Is there a form for this? Thank you.

There is no fool proof fail safe way to obtain what you seek short of adopting the children. However there are things you could try that are likely to work.

For dealing with school authorities there is no form I know of. I would expect, however, that if the gentleman wrote a letter saying he wanted to authorize the teachers to speak with you and that you were acting on his behalf with regard to the children, that would be sufficient.

Medical is more complicated. You could try three Health Care Directives, one for each child. In this document, the father would appoint you as the person to make medical decisions whenever the child is unable to decide or speak for himself - which for a minor might be all the time.
