Name change
Greetings,
I have a cousin who lost his original birth certificate many years ago. He recently recieved a copy of his birth certificate and finds out that his last name is differant than what he has been using for most of his life. It turns out that his father did not sign the original birth certificate. Since he has been using the wrong name for so many years (40), he wants to continue to use it. How can this be done?
Any advise is appreciated.

He will need to hire an attorney to petition the court for an amendment to his birth certificate.
