There is a process to do name changes. There are pro se (do it yourself) forms, but I recommend using an attorney from the start. It does cost some money. But since the child is young, and both parents are seeking the name change, it should cost less than most. If you want an estimate of how much your child's matter will be, you should consult directly Witt a civil practice attorney in your area. Many offer a free or low cost initial consultation.

Good luck

It is not a difficult procedure. Here in Jasper County, where you are judging by your zip code, the court will often allow the name change without a court appearance if both parents consent, It sounds like you have those facts. Give me a call if you would like to discuss.
