Legal Question in Insurance Law in Missouri

I have a daughter from my x that needed wrist surgery. My x just so happens to work in a building that Dr's perform the type of surgery she needed. So she knows them and wanted one of them to perform this surgery. Well normally this would be fine but it's out of network driving the price of out of pocket expenses way up. By the way my wife now carries the insurgence on her policy for my children. I told her that she would need to go to an in network Doctor so the medical bills would be paid. She refused to do this and I did not authorize for the out of network Doctor to perform the surgery. Well my x had it done anyway and now my wife now is receiving a bill saying she owes $900 because the insurance is in her name. Can they do this. I would think that if she wanted to go out of network then that should be on her.
