If I make an item from a copyrighted pattern is it legal for me to sell the item for a profit or must I first have the pattern designer's permission?

Generally, the pattern is what is copyrighted, not the item. I understand your question to be talking about dress patterns, or the like. If you mean something else then you need to make your question a little more informative by adding more details. For example, did you steal the pattern or buy it or what? What is the item the pattern is used to create?
