Adding a name to a deed

I am wishing to add my name to the deed for a house my parents own. They are in agreement to this. What would we need to fill out to file with the County Clerk?

Depending upon what your goals are, and your parents, you may not want to add your name to the deed". You may be better off having your parents exeucte a Beneficiary Deed, in which they name you as the beneficiary to take title to the property upon their death (the death of the last one to survive.)In any event, you do not file anythig with "the County Clerk", if there is a deed transferring interests in real estate it should be recorded in the Recorder of Deeds office. You should consult with an attorney in the county in which the real estate is located.
