Legal Question in Family Law in Nevada

Name Change for my 20 year old daughter

My daughter, who is 20, would like to have her name changed to her dad's last name. Me and her father both consent to this happening. How do I go about doing this?

I got a form from findforms.com, but it is for 21 or older.

Please help.

I did the form you are using. In Nevada you only have to be 18 to change your name and you do not need anyone's consent.

In Nevada you only have to be 18 to change your name. Once you are 18 years old you do not need your parent's consent. You have to follow the proper court procedure, including the filing of the petition, publication, etc.
