Joint Petition for divorce

We have filed a joint petition however the judge has requested an amended joint petition due to a variation in child support. We calculated child support by taken her income *.18 - my income *.18 - insurance and came to the amount. We are now told we need to state that on the amended decree. What is the proper verbage for such and do we initial joint custodians the amount is according to the statue and place the description in or do we initial that the amount differs from the statue and do we put mom or dads income in for the amount that the statue requires then place the description below that. The judge's secretary told me we are correct in how we calculated the amounts we just need to place the right verbage into the decree, but could not assist with that matter.
