Inform your immediate supervisor that you need further medical services and fill out any forms they require to initiate a worker's compensation claim. Your employer is protected from most claims of injury in the workplace by Nevada's worker's compensation statutory scheme, so you cannot contemplate suing the boss without first allowing him the opportunity to make you whole via worker's comp. If your immediate supervisor does not seem to take you seriously, escalate it directly to the Human Resources or Benefits office - or go straight to the owner, if it is a small company - and force them to file the claim in your behalf and get the medical treatments NOW!

Also, get instruction from your doctor as to whether you are capable of returning to work right away, as you may need to take some (compensated) time off to rehabilitate. That, too, is covered by the worker's comp insurance your employer purchases.
