Legal Question in Family Law in New Jersey

My ex husband is supposed to carry health insurancce for our two children. Since our divorce in 2008 he has not. due to him changing jobs frequently and being unemployed he stated he cannot afford COBRA, so i receive NJFamily care for them since 2008. It also states he is responsible for 75% of all medical bills after I pay the first $250. Our daughter just received braces and I sent him the payment schedule. All I want him to pay is two payments of $500 each total of $1000 per the doctors payment schedule. I told him I will pay the $160 per month for the 20 months and he wont have to pay anything after the $1000. The total bill is $4200. He out rite refuses. What can I do?
