raise rent girlfriend moved in without approval
Tenant moved girlfriend in without written approval as stated in the lease. Moved in with a dog, can we raise the rent mostly due to wear and tear the lease clearly states written approval is necessary.

It sounds like you are past the discussion stage. You may want to file suit to evict. I suggest you consult an attorney who is near you who regularly goes into landlord tenant court.
