what form do I need if deceased husband left no will and I surviving spouse with no children or remarried n eeds proof to claim life insurance policy over 20,000 in the state of new jersey

If you are the named beneficiary on the insurance policy, you need only make a claim directly to the insurance company. It will want to see a death certificate and your personal ID.

I agree with Miriam. However, if no beneficiary is named, so policy goes to his estate, you need to apply for and be appointed Administrator of his estate. You might need an attorney to assist you.
