http://www.nmadvocacy.org/ go to this address and you can download a power of attorney form - requires parent's signature. If you can't find parent, download the caregiver's affidavit. This form you can give to a doctor, dentist, hospital, school, to get what you need for child. If you've had the child for 90 days or more with no parent living with you, you can apply for a kinship guardianship. Call Pegasus legal services for children - (505) 244-1101 - they can pay for an attorney for you if you meet income guidelines, or steer you to an attorney or online forms if you don't. Call me if you have questions 505 280-3110.

Good luck. There's a special place in heaven for grandparents who do this.
