I have a question about my mother's durable power of attorney [of which I am the agent], ...

Do I need to sign it?

In the "Affidavit that Power of Attorney is in Full Force" section ?

Please advise. thank you.

Not until such time that the power-of-attorney is in use. For example, it may not be in effect until your mom is incapacitated. You will need to review the POA to see when its effective.
