Legal Question in Appeals and Writs in New York

I want to bring a 17 year old case back to the court calendar on a CPL 440 motion. I have been advised this is the route I must take, by the previuos attorney.

IS there a form I can take to the Supreme Court that can initiate the proceedings?

There is no form for such a complicated procedure. Not to too simplistic but, the motion should drafted with the case caption, titled with the relief requested, and the body should state the reasons why the motions should be granted and it should follow the outline of the law of the relevant statute. In your case, it is one or more of CPL 440's eight grounds.
